This is a story based achievement, and as such cannot be missed.
All characters have to complete the attack on the Academy, resulting in a conversation with Fenthick in the barn. You'll know if you've finished this conversation as you will have levelled up. After this, and the scene transition, talk to Lady Aribeth and the achievement should pop.
