Basically just declare war on a enemy civilization and conquer all their cities, this will remove them from the game and you will get this. It's obviously easier on a low difficulty game, the earlier you do it the less cities will be needed to be conquered to eliminate them.
you will also get a ending talk from the civilization leader, which is phrased a bit different for each civilization.
