Once you spawn into your toilet, don't touch the toilet paper in the bottom right.
Turn to the left to see a toilet drawn on the wall. Continue to press that toilet and you'll get a random item each time. The developer stated that there's a 5% chance to get a legendary item, and this is one of them.
