When in the spear fishing minigame, you can't make a single mistake, need to catch all 3 fishes in a row. Luckily from what I saw in my playthrough the fishes will come again if you can't get your crosshair on them in time. So just get all 3 fishes and the achievement will unlock after a few moments.
