To trigger a death route, you have to make it through the level up to a certain point without dying. All you've got to do is make it to the death route pad without dying, then jump on the pad to be transported to the route.
The following levels have a death route:
Air Crash
Road to Ruin
Diggin' It
Cold Hard Crash
Piston It Away
Night Fight
