In No Way Out mode, bring Lillian's item (middle grave) to it's location, then head towards either the shack or the bus, dropping your candle along the way. It seems that she is more likely to play with you if you're in the dark. Stay near one of those places, heading back to the nearest one every time she teleports you. When she starts counting, go inside to get the achievement.
