In the No Way Out DLC, first place Tim's item (on the far left), then Toms (next to it). Once you've placed Tom's item put down your candle and wonder around in the dark until you get teleported to the church. You can use some matches if you want. It may take quite a while wondering around in the dark before you get teleported, so be patient.
