This achievement is story relevant and can't be missed.
*** Spoiler - click to reveal ***After getting access to Laverne, have her ask the guard to go to the bathroom. Flush her scalpel to Bernard. Go with Bernard through the double doors in the back of the starting room with the clock and have him use the scalpel on Oozo the inflatable clown.
