Grandest Campaigner requires (as I've been told on Slitherine's forums) completion of *both* West and East Campaigns.
Nationality Victory achievements unlock at the end of the game, but does not necessarily follow the requirements as they state it. You can buy units of any nationality as long as it is done before deployment (bug), and it sometimes won't unlock if you don't win the campaign decisively - losing or gaining a minor (normal) victory win = no achievement for whatever reason. So I put them down as bugged.
All the ones I've marked as bugged were because I gained them for no discernable reason, they may have been patched, I'm not sure I need someone who doesn't have them to confirm. I basically started a new game and they all popped for me at the wrong points. =x
