I got this one at the first wave following my crafting of the Elixir of Fireball.
Had a bunch of skeletons and zombies small size into a narrow tunnel, and launched muscle and fire power into the battle all at once.
I had not even noticed it popped...
Again, this happened during the 1 level of campaign.
