This achievement can be missable.
After completing the inquisitor's questline with the 3 dark organs, return to him the next week. He will say he needs more proof of evil and three dialogue options appear. One is to tell him about Gerry, this is the option to choose for the achievement. All options end the conversation and you will miss the opportunity if you dont choose correctly.
