How to unlock the ^**5%_=+12 achievement

  This achievement is for beating the glitch level in Hell. Glitch levels are unlocked by first beating the boss in the world you are in, and then replaying levels until you see a glitchy bandage girl. Upon completing the level you will unlock the glitch level for that world.
