You need to collect all bandages in the game (100). These are found in every world besides "The End" and "Cotton Alley". There are 2 bandages in every purple warp zone. Bandages can be found in light world levels, dark world levels, and purple warp zones. Look up videos on where each bandage is if you're having trouble.
