This is proof that you've acquired a piece of equipment with a +99 modifier. Sanctify it, and a whole new life awaits you. Aim for 9900 fusions! No, wait, that's probably not necessary.

How to unlock the Last Stop on the Fusion Train achievement

  If you just want the achievement and don't care as much about making a good and usable piece of equipment, then use one of the joke equips for this. I used the Languid Sword that you get from an optional boss in Camlann. Then buy all the swords you can and fuse them.
  
  Once you get +99 the achievement unlocks.
