This achievement can be found in level 1 "Trial By Blood".
At the start of the level you will be in an elevator and receive a call from big Jim do not interact with the walkie talkie on the body bin. Instead wait in the elevator for at least 1 minute.
The elevator will collapse and the achievement will pop.
