The description of this achievement could be interpreted as a little misleading as you don't actually have to replay the whole game on Hard mode.

Instead, if you've finished the game on Normal mode, reload your save and simply select "Hard" at the bottom of the map screen and replay the last chapter again.

Your goals for this chapter are:

    - 10 "Perfect" hands - meaning you'll need to earn 3 stars on each hand.
    - Collect 6,000 gold - this is cumulative, and won't be an issue..
    - Make a combo of 35 (and earning the Combo Countess achievement.

There are only two hands that allow a 35 combo streak: hands 7 and 10 - hand 10 is probably the easier of the two. Play through each hand whilst saving as many wildcards as possible (the more Joker wildcards you have, the better) and stockpile them. Feel free to burn through the first 9 hands as these aren't that important.

Once you've reached hand 10, try and aim for the 35 combo. If you get stuck mid-combo, play a wildcard, but be sure to plan your route in advance; you should have 11 wildcards saved up, so don't be afraid to use them. If, after playing your wildcards and you're still stuck, don't be tempted to use your Cupid or Phoenix power-ups as there won't be enough cards left to earn a 35 combo. Using the Harp is fine as this merely shuffles the cards on the table.

All-in-all, it's not the hardest of achievements, and if you plan ahead, shouldn't cause you too much of a headache.
