You get this simply by beating the final boss of the game in North Crater. You have to fight Jenova's final form, Sephiroth, and then Sephiroth's 'final form'. However, once this is done, you will then have to go through the 'battle' at the end with Sephiroth where Cloud uses Omnislash (regardless of whether you have it unlocked) before the achievement will pop.
