This is done easiest on the 'Intensive Treatment' challenge map, as it has the least enemies and no knife fighters or stun batons. Simply complete the challenge without getting hit, and keep in mind that you can break combos, so if you need to run to avoid a hit it won't count against you.
I recommend using the evade maneuver and throw move a lot for crowd control. This game annoyingly likes to have as many goons swarm you as possible, and unlike later games you can only counter one enemy at a time, so if it looks like you will be attacked by more than one goon, move away as quick as you can.
