The hammer required for this achievement is the Pwnhammer. This hammer is a guaranteed drop from the Wall of Flesh boss, which is summoned by throwing a Guide Voodoo Doll into lava.
After getting this hammer go to your corruption/crimson biome and use it to destroy an altar. If you try to do this in a world where the Wall of Flesh has not yet been defeated you will take damage instead.
