First, make sure the game you are going into has the rise and fall dlc enabled.
A heroic age will occur when you achieve a golden age, while you were previously in a dark age.
To do this, just make sure that in one era you do little to nothing with era score. Then, when you get a dark age, focus everything into getting era score.
If successful the achievement will pop at the start of the heroic age.
