Easy way to do this - set a undead ork as your bodyguard. Undead orks don't turn on you no matter how many times you hit them. Beat the bodyguard up untill he is bleeding near an Olog - I did it in the Gorgoroth keep. Wait till the ork bled 3/4 and ride the olog. If he dies while you are riding you will unlock the achievement.
