To play as the Gangster (Blahd), and thus unlocking this achievement, you'll need to kill 10 Crepes (gangster in blue).
Note that this achievement isn't cumulative across multiple playthrough, so you'll need to kill all 10 in one playthough. Gangsters of both varieties are common throughout the levels, but can be mostly found hiding down dead-ends or in small houses.
