You will earn this achievement for completing a side quest related to Laphicet for creating the Omega Elixer for Videl. This is a part based quest where you are unable to move forward until you hit specific parts in the main story.
In Reneed, after Melchior escapes from Lothringen, there will be an ! by two kids and the dog at the north of the town. After talking to them, go towards the inn and talk to the boy reading to trigger a scene. You will be tasked to find the ingredients for the Omega Elixer. The first Ingredient will be in Vester Tunnels in Kurogane's workshop. Examine the turquoise crystal to collect it. Afterwards, head back to Reneed and talk to the Videl nearby the inn.
After taking Medissa back to Titania head back to Figahl Icecaps and examining the sparkling point at the east side on the secret way to Hellawes. Return to Reneed and talk to the Videl.
The next item is available right away. Head to Eastern Galles Lake Road. Look at the mini map to find the exact location of the item, it is marked with an !. Its on a huge island that is it's own area. Alternatively this can be obtained at the Baird Marsh later in the game as well. After getting the item return to Reneed and talk to Videl's Mother in front of the inn.
The 4th item will not be available until after you wake the 4 Empyreans and arrive back at Port Zekson. You can then teleport to Stonebury and head north towards the Davahl Forest. The last item will be at the end where you had fought the Serial Killer Tree Code Red Daemon. There you fill find an Exorcist who has found the 4th ingredient and gives it to you. He will also tell you the final ingredient which is.
Head back to Reneed and speak to Videl's mother to wrap up the quest. At the end of the cutscene the achievement will pop.
