Note that getting this achievement will not also net you the Survivor and Persistent Survivor achievements for playing all the way through from 1066 and 867 respectively. You will have to start separate games at those start dates to get them.
To get this achievement, simply survive until the end of the game in 1453. Choose a powerful, stable dynasty (Abbasids or Charlomagne are good) to start and you should be fine. If you're still pretty new to the game then marry your males to princesses as high up in other succession lines as possible and invite foreign princes to your court and set up matrilineal marriages to your female dynasty members to increase the spread of your dynasty as much as possible. It's unlikely you'll be in any real danger after a couple hundred years so you can then focus on whatever else you want to in the game.
Also note that the game slows down a LOT after the 1000s or so so unless you have a pretty powerful cpu, the last couple hundred years especially will be a total slog.
