  In order to unlock this achievement, you need to score Bronze or better on the Slayer's Gauntlet Challenge Tomb in Score Attack mode. It was really easy to accidentally break my streak on this one because so much of the tomb is "on rails" so to speak with the floating raft - this video by TheTreeble can help:
