During mission 3 you are are given the choice of curing or killing Hypatia. This achievement requires you to cure her. Follow the markers to create the serum then return and use it on her.
After the mission is over and you find yourself back on the Dreadful Wale, simply find Hypatia in a nearby cabin and talk to her to unlock the achievment.
