Simply kill 40 ghouls, got this while cleaning out the ferry zone, I don't think you can miss this and I wouldn't be surprised if you can kill the same ghoul 40 times and just reload in between, that however is wasteful because you will end up killing a lot more than 40 ghouls before completing the campaign.
