To get the achievement, you have to perform a Jump Pass and it should then be converted granting you an assist.
Passing midway through a shot (Jumping to shoot and then passing) doesn't count. The action defined in the controls section must be performed. So you have to press shoot and pass at the same time.
Also each player has his own counter so make the passes with the same player.
