After passing the second vision of the woman(where you need to roll a red boulder to get up to her on the tall rock), you'll come to the only cave in the game. Near the end of the cave there is a singular, glowing mushroom. It's very hard to miss. Click on it, and the glow will disappear, as if though you flicked off a light switch. The achievement will pop.
