You can get this in the 3rd chapter of the game ("The Day After") while playing as Lucas. When you first start the chapter, get out of bed, then take the pills on the bedside table. Exit the bedroom and head to the kitchen. Go the fridge (don't open it), then turn around and you are given the option of getting a bottle of alcohol from beneath the counter. Drink it and you'll die from mixing the pills and liquor and get the achievement.
