During the Adventure Mode part of the story (act 2), turn left at Ticket Lake and enter the level at Weeping Hollow. There's not much to it, just grab the floating coin.
Now go all the way back to the "welcome" banner where you started. Click the square in the bottom-left of the screen to reveal a hidden trail heading west. Keep clicking the squares and board the boat at the end to change your avatar.
