  If you fail a QTE which will result in you dying, quickly pause the game and quit to menu. Then simply load up the chapter you were on (MAKE SURE TO AGAIN SELECT RIDICULOUS DIFFICULTY!!!) and once you finally reach the end, the achievement will pop (along with finishing the game on ridiculous difficulty)
  
  
  This may be patched eventually, but it is a very easy way to earn the most difficult achievement in the game.
