  If you want to get this at the very start of the game, its actually a simple process. Once you leave the tutorial and head out of the castle, cast haste and run down the stairs and eventually you will collect enough fall damage running u and down them that you will die. You can also fall of a cliff at any point in the game.

  Apart from the obvious ways, you can apparently get this achievement by being blown away by a bomb in Chapter 2 (but obviously you need to survive the explosion to get thrown into the air and get killed by the fall). So do not actually sit on a bomb, but stay close enough to fly and you should do it.
