If you're aiming to get this achievement in a non-speed run, it's easy to get in this safe, but slow, manner:

1: Hide in the corners! The Empress Spider's slam attack can't touch you in either corner.
2: Shoot your gun! Yeah, it's a mere 1 damage per hit (unless you upgraded it, up to a 5 damage!)
3: Hair-whip when she slams closest to you! (Hopefully you've grabbed at least a couple hair upgrades, especially speed!)
4: Jump the flames! When she drops a fireball, get to the more open side and be read to jump and then glide over to where the fireball was dropped. She drops only 1 fireball until she hits low health then she drops 2 (not simultaneously).
5: Watch out for the eggs! If you are firing your gun, you'll get rid of them for certain. But if they do hatch, the baby spider is too short for the gun to hit so you have to crouch and hair whip them. She drops 2 at a time until she hits low health then drops 4 at a time (which is not harder at all if you're shooting them).
    6: Repeat until you win.

(Super) Pike Ball is an option... but since most of the time the spider won't be in range you're basically just wasting it. (Remember, this is the paranoid version of the fight!)

Alternatively, you can totally go Hair Whip + Super Pike Ball and try to predict her movements to do max damage as fast as possible, but you really have to be careful to watch those slams, especially when she speeds up in the second half.
