  Play against bots on Easy. You'll automatically start with the bomb in hand, so look towards one of your teammates and throw it to them with G - now they have the bomb! Wait for them to get killed during the round, then pick it up and plant it.
  
  Not sure if it's required, but the achievement didn't unlock for me until the bomb actually exploded! So make sure the CT team isn't annihilated or defuses the bomb!
